CMS' Making Care Primary model applications are due Nov. 30: 7 things to know

The application deadline for CMS' new primary care payment model is Nov. 30.

The model was developed to improve care management and care coordination and connect primary care physicians with resources to develop partnerships with healthcare specialists, according to the CMS website.

Here are seven things to know about the model before the application deadline:

  1. The multipayer model will launch July 1, 2024, and will run through December 2034.
  2. The model includes three participation tracks to build on previous primary care models: building infrastructure, implementing advanced primary care and optimizing care and partnerships.
  3. The model will be available in eight states: Colorado, North Carolina, New Jersey, New Mexico, New York, Minnesota, Massachusetts and Washington.
  4. Only practices north of Westchester County are eligible for the model in New York.
  5. Applicants must have the majority of their facilities located in an eligible state.
  6. Applicants must have at least 40% of their Medicare service revenue come from primary care services.
  7. Applicants must have a minimum of 125 attributed Medicare beneficiaries.
