CMS will launch a pilot value-based primary care model across eight states through the Center for Medicare and Medicaid Innovation, also known as the Innovation Center.
The Making Care Primary Model will be tested from July 2024 to the end of 2034 and will aim to create more coordinated care for rural and underserved communities, according to a June 8 news release from CMS.
"Primary care clinicians are the first line of defense for prevention, screening, management of chronic conditions and overall wellness," the release said. "Through MCP, [the Innovation Center] increases the investment in primary care so patients can access more seamless, high-quality, whole-person care."
The model will offer increased payments to participating primary care organizations and practices, particularly small, independent, rural and safety net organizations and other organizations with limited experience in value-based care.
The participating states are Colorado, Massachusetts, Minnesota, New Jersey, New Mexico, New York, North Carolina and Washington. Applications for the program will open in late summer.
