AMA provides key operations forms for private practices

The American Medical Association recently published an appendix to its "AMA STEPS Forward Private Practice Playbook" that includes business and medical forms physicians can use in their private practice.

Here are five forms the AMA recommends to help physicians manage medical practice business operations:

  1. New patient packets that include patient history, surprise billing and telehealth consent forms, along with health plan information, beneficiary forms, and privacy forms and HIPAA documents.
  2. Patient documents that include information on practice financial policies, payment plans and price lists.
  3. Administrative documents with medication logs and daily payment forms.
  4. Employee documents that include employee performance reviews, time off and expense reimbursement forms.
  5. New hire documents with employee consent forms and other records like drug screening, payroll deduction and vaccination requests.
